Terms of Use of the Copayment Program

This Copayment Program (hereinafter the “Program”) is owned by AstraZeneca CAMCAR (hereinafter “AstraZeneca”). By using this Program, you agree to these terms of use. You cannot join this Program if you do not agree with these terms of use.
AstraZeneca reserves the right, at its sole discretion, to change, modify, add, or remove portions of these Terms of Use at any time, without any liability whatsoever to AstraZeneca. You must review these Terms of Use on a regular basis to confirm whether they have changed and remain in effect.
Nothing stated in this Program should be construed as medical advice or recommendation, nor should it be used as a basis for making decisions or undertaking self-medication. All medical advice should be provided by healthcare professionals.

Copayment Program

1. It will only be applicable in enrolled and duly authorized pharmacies.
2. The copayment coupon corresponds to a fixed amount covered for the purchase of each treatment previously pre-approved by your health insurance.
3. The copayment coupon is valid for a single redemption per patient per authorized Product in the country of permanent residence of the relevant patient.
4. The copayment coupon applies for a fixed discount on the quantity of treatment medication indicated by the nurse in charge when enrolling you on the platform.
5. The copayment coupon cannot be combined with other AstraZeneca coupons.
6. The copayment coupon will be generated with the support of the Celebrate Life Program nurse.
7. The term of the Program will be indefinite. However, AstraZeneca reserves the discretionary right to unilaterally terminate the Program, with the sole obligation to inform the interested parties and without incurring any liability for such termination. In the event of program cancellation, AstraZeneca will honor any unexpired discounts.
8. The copayment coupon cannot be combined, under any circumstances, with other AstraZeneca discounts and/or redemptions and is valid for a single copayment. The copayment coupon is for personal use by the holder of the identity document associated with it. The copayment coupon is non-transferable and cannot be used by third parties. In addition, the copayment coupon is non-refundable and cannot be exchanged for cash or any other type of discount or copayment coupon.
9. The copayment coupon cannot be redeemed through the own e-commerce platforms of authorized pharmacies; therefore, the redemption must be made in person at authorized pharmaceutical establishments. The coupon discount will not apply to shipping or associated costs.
10. The authorized pharmacy is responsible for verifying that the identification document corresponds to the one assigned to the coupon in the system, the validity of the copayment coupon, its effectiveness (not expired), and for redeeming the applicable copayment coupon. If a copayment coupon has already been used on previous purchases, an error message will appear when applying the discount at authorized pharmacies.
11. The copayment coupon cannot be combined with other AstraZeneca discounts or redemptions and is valid for a single redemption by the holder of the ID associated with the coupon. The copayment coupon will only be valid for products previously authorized by AstraZeneca.
12. The copayment coupon is valid for redemption from the moment it is created or downloaded until the expiration date shown (30 days after download).
13. It is strictly prohibited to sell or trade the assigned copayment coupon.
14. AstraZeneca will not be liable for any misuse, unlawful or unauthorized use of personal information, as AstraZeneca will not collect or store any personal data associated with these coupons.
15. Patients must mandatorily accept the Terms and Conditions. Also, you must present the coupon along with your health insurance pre-approval at enrolled pharmacies.
16. Valid copayment coupon: one (one coupon) per associated product per calendar month.
17. Copayment coupons will only be valid for the countries and brands detailed on the website www.celebratelifeprogram.com.
18. AstraZeneca reserves the right to limit the number of coupons available at its discretion.

20. Coupons are developed by AstraZeneca for use by patients only after a prescription has been given by a treating physician and pre-approval has been obtained from their health insurance.
